General information for applying to graduate school:
The first step to applying to graduate or professional school is to research on the school's admissions deadline. This is so you can give yourself enough time to gather all the required components to submit your completed application in a timely fashion. Most graduate and professional schools require the following:

Letters of recommendation
Personal Essay
Standardized tests (e.g. GRE, GMAT, LSAT, OAT, MCAT)
